The Undersea and Hyperbaric Medical Society (UHMS), one of the leading professional organizations in hyperbaric medicine, currently recognizes 15 clinical indications for hyperbaric oxygen therapy. The FDA also regulates hyperbaric chambers as Class II medical devices and clears specific devices for medical use.
Understanding these established applications provides important context for anyone exploring HBOT.
What Is Hyperbaric Oxygen Therapy?
HBOT stands for hyperbaric oxygen therapy.
During HBOT, a person enters a pressurized hyperbaric chamber and breathes medical-grade oxygen according to a prescribed protocol. Increased atmospheric pressure allows more oxygen to become available to tissues throughout the body.
That ability to increase tissue oxygenation is one reason HBOT has become useful in several very different areas of medicine.
What Medical Conditions Is HBOT Used For?
The current UHMS list of recognized indications includes:
- Air or gas embolism
- Carbon monoxide poisoning
- Carbon monoxide poisoning complicated by cyanide poisoning
- Gas gangrene
- Crush injuries, compartment syndrome, and other acute traumatic ischemias
- Decompression sickness
- Central retinal artery occlusion
- Selected problem wounds, including certain diabetic wounds
- Severe anemia
- Intracranial abscess
- Necrotizing soft tissue infections
- Refractory osteomyelitis
- Delayed radiation injury to soft tissue or bone
- Compromised grafts and flaps
- Acute thermal burns
- Idiopathic sudden sensorineural hearing loss
- Avascular necrosis, also known as aseptic osteonecrosis
Some of these are grouped together within UHMS categories, resulting in 15 primary recognized indications in its current 15th Edition.
These conditions may look unrelated, but many share a common problem: tissue is not receiving the oxygen it needs, or increased oxygen can play an important role in supporting medical treatment.
Why Is HBOT Used for Wounds and Damaged Tissue?
Oxygen is essential to tissue repair.
This helps explain why HBOT has an established role in conditions such as certain diabetic wounds, radiation-related tissue injury, compromised grafts and flaps, severe burns, and traumatic injuries where circulation has been disrupted.
For example, after reconstructive surgery, a skin graft or flap can occasionally become threatened because it is not receiving adequate blood flow. HBOT may be used as an adjunctive treatment when tissue becomes hypoxic or compromised.
That does not mean everyone undergoing surgery needs HBOT. Its established medical role is much more specific.
Is HBOT Covered by Insurance?
Sometimes — but coverage depends on the condition, medical necessity, insurance plan, and treatment setting.
Medicare, for example, covers HBOT for certain qualifying conditions such as carbon monoxide poisoning, decompression illness, gas gangrene, specific diabetic wounds, compromised skin grafts, refractory osteomyelitis, and radiation tissue injury when its individual criteria are met.
Being discussed or researched as an application for HBOT does not automatically make a treatment insurance-covered.
What About HBOT for Wellness and Longevity?
Beyond its established medical indications, researchers are studying hyperbaric oxygen in additional areas including recovery, neurological health, healthy aging, post-surgical support, and other emerging applications.
This is where an important distinction matters.
An established medical indication has a different level of evidence and clinical guidance than an emerging application that is still being investigated.
